I have a question. My grandma has a "list" that is like a grid system that has different collumbs: Name, Pain level, hours worked etc.. I want one of those lists, but I cannot seem to find one. I appreciate any help you can provide.

Tyler, the Form you seek is the 60-Hour Session Documentation Form.

Here is how to find it:

Go to Quantum Touch Home page: http://www.quantumtouch.com

Scroll to bottom of page, looking on the left side of the page.

Under PRACTITIONERS on the left side you will see "Practitioner Forms." Click on that.

There will be a list of available forms. The one you want is 60 Hour Session Documentation Form in English PDF version.

Click on "More" in that box.

On the next page, click on "Download now."

The form will come up for you to save or to print. You can save it and print as many copies as you need to document your 60 hours.
